Overview
Human recombinant bone morphogenetic protein-2 (rhBMP-2) is frequently used in skeletal reconstruction in clinical practice, but therapy with supra-physiological doses for stable bone formation induction causes side effects such as ectopic bone formation and activation of osteoclasts.

This challenge reflects the lack of an adequate carrier that can maintain sustained release of a certain amount of rhBMP-2 for a long period of time; instead, it is necessary to administer rhBMP-2 at a high concentration to obtain sufficient bone formation-inducing effects.

Therefore, the authors used a rat calvarial defect model to evaluate the dose-dependent signalling mechanism of rhBMP-2 and suppression of bone resorption (side effects of rhBMP-2) by using a nuclear factor of activated T cells (NFAT) inhibitor.

For sustained release of rhBMP-2 and NFAT inhibitors, a collagen sponge prepared from Atelocollagen Powder and chondroitin sulfate was used.

The authors showed that the expression of bone resorption markers was elevated in the group administered with high-concentration rhBMP-2 alone. In contrast, expression of the same markers was decreased, and bone resorption was suppressed, in the rhBMP-2 and NFAT inhibitor combination group.
